Medication errors are one of the most common, and most preventable, causes of hospital admission for older Australians. Polypharmacy (multiple medications) makes the risk worse, and many clients are taking 5-10 medications at any time.
We provide structured medication support: prompts, administration when authorised, observation for side effects, and regular review with your GP and pharmacist. If something doesn't look right, we say so.

We track the things that quietly indicate decline: changes in weight, sleep patterns, skin condition, continence, mood, mobility, and cognition. These observations get logged, reviewed by a Registered Nurse, and shared with families and GPs as needed.
